You are considered enrolled when full payment or purchase order is received. Substitutions for pre-registered persons will be accepted. Please copy or print the form for additional registrants. Payment is expected prior to the date of the course unless other arrangements have been made with the course coordinator.
Confirmation and payment information will be mailed upon receipt of the registration form(s). Classes are subject to cancellation or change depending on enrollment.
The registration fee (less 25% cancellation charge) is refundable if a written cancellation notice is received at least two weeks prior to the first day of the course. If you are unable to attend, a substitution can be made at any time at no extra charge. The University reserves the right to cancel programs if necessary. In this unlikely event, the University’s liability is limited to the full refund of your registration fee.
Enrollment is on a “first come, first served” basis. If your registration is received after a class is closed, we will contact you about the status of the class. Also, we can register your trainee(s) in the next available class or refer you to one of our Consortium members who will be offering the class of your choice.
